Our end of the season banquet/ awards night is on Thursday, June 7 in the cafeteria @ 6:00pm. Everyone needs to attend.  

Please return all forms together in an envelope with the athletes name on it.

Team Checklist- This form is to make sure that you have submitted all you necessary forms and a parent email address for us to have.

Team Fund- This is a one time fee that will cover all team cost for the season.  If for some reason there is some type of hardship, please contact Coach Culver for arrangements. Please read the form.

Team contract- Please read carefully.  Parents and Athletes must sign this form.  Just the signed portion needs to be returned.

Donation Form (Optional)- Please read form. This is optional and greatly appreciated if you do decide to donate.  Any donation helps our team financial needs.

Booster Form (Optional)- Please read form. This is optional, but if you have not already joined, I encourage you to join as the boosters help with all financial needs for Centreville sports.  

1. VHSL Physical Form.  Must be completed and turned into the athletic training office.  If you already have one completed and turned in than another one is not needed.  The form can be found in the Team Files Tab.

2. Concussion Education.  Must be completed by the parent and student athlete. Once again both parent and student athlete must complete. If you participated in a fall or winter sport than you do not have to complete it again.  The link is in the Team Links Tab.

3. Emergency Care Form.  Must have a new form completed for each season.  Turn form into Coach Culver

4. Participation Form. Must have a new one for each season.  Turn form into Coach Culver
